First off they are under staffed but they were polite even though the patients were not polite the room I was placed in had a leak I toilet for two weeks and i couldn't use it without walking through it and had to place towels around iti wanted to leave after the 20 days I was supposed to be there they claimed that I had a wound VAC so I couldn't but the first 7 days I had no machine so why not just let me leave after the 20 days and remove the VAC like they ultimately did anyways because if it was so important to have on I was placed here after having my big toe removed from the hospital I was told that they would have a wound VAC machine available before I left the hospital they didn't have the machine and it was not available they removed my wound VAC bandage after two hours of being there and they had just put it on at the hospital prior to being sent there it caused me to start bleed after two hours of bleeding they freaked out and couldn't get it to stop bleeding so they called amr to send me back to the hospital for 1800$ bythe time amr arrives the bleeding stopped,so they could have just used medical transportation that was covered by Oregon care insurance the hospital just put bandage on and sent me back to this nursing facility it took over a week to get a wound VAC machine that was required by the hospital as part of my post recovery they changed the bandages irregularly not constantly waiting times for medications were hour's passed due the patients were yelling and screaming for nurses without resolve ultimately I left because of the inconsistentcy and rude patients

I got to this facility June 19th was Discharged July 26th... My insurance only covered 20days i told them I couldn't do private pay, Yet I was kept there after the 20 days because Nursing wouldn't Discharge me... Only 3 people were a decent help to me one was a charge nurse who was Agency worked the weekend, One was OT and one was PT I was a CNA for over 15 years A lot of those so called CNA's shouldn't even have a license... I saw a Actual Dr 1 time in the entire stay... Since I got home in July took over my own wound care my wounds have fully healed I did not need to go to a wound clinic the facility claimed I needed to go to, I was told by the facility I needed Minor surgery due to hypergranulation or that my wounds would not heal which was not the case.. movement, Circulation, Blood flow that is what has healed my wounds completely, This facility lied to me, There was a time when I needed assistance using a bedside toilet I had pressed the call light waited 40 minutes before a Day "CNA" That was assigned to me finally came in to do his Job, This facility runs off Agency Staff who alot of the time.dont know what theynare doing at all very rare did I see anyone that knew what they were doing.... Part of my healing process was The need for Protein and even though Double portions of food were ordered i didn't get what was ordered so I had to take matters into my own hands and Have food delivered to me to Eat properly for what I needed... Put youre loved ones here st youre own risk, Or if you are a patient be here st youre own Risk... I was kept here for money and to fill a bed nothing more they tired to help me sign up for another insurance thing and when I was denied then I was discharged the next day... In all my years of Healthcare experience I would give this place the biggest negative rating humanly possible, as a Patient those 3 Agency people that helped me are they only reason you got a 1 star... My wounds were not making any progress here at this facility, when I took over my own wound care at home after finally going home wounds healed just fine.... So many dishonest and uneducated nurses and staff...

If you are reading this and are the kind of person that stays on top of your family members' care, it will not be enough. This facility has dropped the ball so many times I won't have enough space on the review to write them all. From taking my father's belongings without even asking to my father having to physically call 911 for help because no one came to check on him when he had fallen on the floor with the door closed to his room and no attendant to check on him for hours. All they will do here is continue to apologize for every mistake and do very little to change the situation. My father is diabetic, and in one phone conference they were concerned about my father's blood sugar but when you are feeding my father pancakes and cream of wheat in the same meal it should be common sense that you are the problem. Even after changing his dietary plan, they were still giving him carb heavy food. My only recommendation for anyone who has a family member staying here because of the lack of options through Medicare is to make sure you are constantly on top of the staff, making sure they are following up with them. The constant lack of communication is what I kept finding between their staff. I felt on multiple occasions that if you didn't tell the staff what has happened or going on with your family member, they won't know or communicate it with each other. Lastly, you will expect to very seldom to ever have anyone answer the main line, especially if you are waiting to be buzzed in.

If you have a choice in a facility that performs the care for your family member, I would not recommend this facility but if you are left with no choices, then I would highly advise that you are constantly staying on top of these people as they will not give the care that may be satisfactory to you.

There are individual people who are exemplary, however they are let down by a larger organization that seems (at best) disorganized or incompetent. Very few of the day to day people you will meet will actually work for EmpRes and it shows. The contractors never take ownership, and you can push the call button at night and wait 30+ minutes for a reply. The facility is noisy during the day which isn't the most restful environment. TVs blare all hours of the day, a few of the longer term residents make disturbing noises at high vocal volume and pitch, and the seemly constant parade of loud wheeled carts and bins will really get on your nerves fast.

But the toilet facilities. I swear the whole month I was in there the bathroom was cleaned maybe once, and far from thoroughly. It reeked of urine, and I had difficulty using it with the overwhelming smell. 4 or 5 people to one restroom seems very problematic even if you are attentive and the "users" make an effort. But with disabled and sick people you can't expect the best of them to keep a bathroom clean. Staffing issue here?

Oh and food isn't great. It was cheap food mostly. And they couldn't quite figure out a renal appropriate diet for me. Every meal seemed to have potatoes. Some meals also had a fruit cocktail full of high potassium fruits. You want something to drink? Better hope you're trying to not drink citrus (not a renal safe juice) or have dairy (also not appropriate for someone on a renal diet). No alt milks. Sometimes the "alternative" meals aren't better.

So basically not horrible, but far from even a 1 star hotel.

Oh and I should add I picked up a nasty bedbug infestation on my most recent visit. Even after specifically requesting my bedding be given a thorough inspection I still came back home with the arachnid gift that keeps on giving.

I am a healthcare worker. I have worked in the health care field for 30+years. From hospice to one on one..I have worked at some pretty awful places. But last October I was placed in here after Knee manipulation surgery following a total knee replacement surgery.. They treated me wonderful. The Staff and cooks, administrators. All so welcoming.. They get busy like most and there is a wait for things..
But not a long wait..I know the business so I am patient even when I am in pain..I know they will come in and address all my concerns. .I never felt like I couldn't tell them what I needed. .It felt good knowing I was in such great hands..Now we understand we can't all be perfect 100% of the time..
But being a caregiver myself. . I know firsthand..It's hard work and you get very little praise. .So I say to the hard working people at this facility. Job well done. Thanks for your Service..

The professional staff at Portland Health and Rehab is excellent — both skilled nursing and physical/occupational/speech therapy. A special shout out to the activities director, Dakota, as well. He is AMAZING!

The missing star is what my mom reported as general disorganization and lack of communication among caregivers when they changed shifts, especially in terms of knowing where her clothes, support stockings, pajamas and personal belongings were.

I can also verify that she sat in one chair most of the day and that I had to actively work with staff to keep her call button and phone within reach, her feet elevated, and to get her up and down out of her chair on weekends when PT was unavailable. We also experienced delays in discharge due to the sheer volume of patients being discharged — no fault of the discharge planner Tonya, who was also excellent, but rather the fact that the facility seemed overwhelmed and understaffed.

With that said, I — as my mom’s daughter and POA — was accepting of the trade-off!

❤Best compassionate people to work with❤
Everyone makes you feel like family. We have very hard workers who genuinely care for each other and the residents. I love how everyone bands together to get the job done, and supports one another from housekeeping to dietary. The cna's are top notch, really care and go above and beyond. The nurses are amazing and extremely knowledgeable. Activities works so hard to bring families in to visit thier loved ones and make things fun. Its an amazing place to be. It's been tough for our little place during this national pandemic. Lot's of loved one's were lost to a horrible virus. Yet the spirit and resiliency shines through and we just get stronger. Im personally very proud of each person here, including our residents, and thier families who are so kind and supportive?
